Take the following instrument 
and picture on a new layer several lines. You must get the same thing like in the picture.

and create the text menu with the parameters from below.

The divisions between the text are possible to create using the instrument. Make a vertical line with 1 px width. Then paint the markings with the instrument 
and light color. Make one more line, but this time of dark color. Take the instrument 
and erase the ends of our lines. The horizontal lines we’ll make different from the vertical ones.
Take the same instrument and draw horizontally the lines. Paint them, mark a greater part and gradient the marked places, using the instrument

Insert the text about the page’s name and the information about you, like it is done in the picture.
